Notice of Privacy Practices (HIPAA)

This notice explains how your health information may be used and disclosed, and how you can access it. It is required by the Privacy Regulations under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We are dedicated to protecting the privacy and confidentiality of your health information and are required by law to do so.

Use & Disclosure of Your Health Information in Special Circumstances

We may use or disclose your health information in the following situations:

Public health & oversight: To public health authorities and health oversight agencies authorized by law.

Legal proceedings: In response to a court or administrative order, subpoenas, or similar processes.

Law enforcement: If required by a law enforcement official.

Serious threats: When necessary to reduce or prevent a serious threat to your health and safety or that of others; disclosures will be made only to someone able to help prevent the threat.

Military: If you are a member of U.S. or foreign military forces (including veterans) and disclosure is required by authorities.

National security: To federal officials for intelligence and national security activities authorized by law.

Custody/incarceration: To correctional institutions or law enforcement if you are an inmate or in custody.

Workers’ compensation: For workers’ compensation or similar programs.

Your Rights Regarding Your Health Information

Access, Copies & Complaints: You have the right to review and obtain a copy of the information we use to make decisions about you (such as medical and billing records; psychotherapy notes are excluded). If you believe your privacy rights have been violated, you may file a written complaint with our practice or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

Preferred communications: You may ask us to communicate with you in a specific way or at a particular location—for example, at home rather than at work. We will accommodate reasonable requests.

Restrictions on use/disclosure: You can request limits on how we use or disclose your information for treatment, payment, or health care operations. You may also ask us to limit disclosures to certain people involved in your care (such as family or friends). If we agree to a restriction, we will follow it except where required by law, in emergencies, or when the information is necessary to treat you.

Amendments: If you believe your information is incorrect or incomplete, you may request an amendment (for records we maintain). Requests must be in writing and include a reason for the change. Send your request to the HIPAA Privacy Officer at the address and phone listed above.

Authorizations: Uses and disclosures not covered by this Notice or by law will require your written authorization.
