Summit Orthopaedics physicians specialize in all aspects of orthpaedic care and can often provide same or next day service to your patients, if needed. Thank you for trusting the Summit team with your patients and we hope to be an excellent partner for you in their care.
To refer a new patient, please print and fill out our Provider Referral Form, and fax it to 208-227-1085.
If you need a consult immediately or to speak with one of our providers, please contact the front desk at 208-227-1100 and let them know you are a local provider and need immediate assistance.
